- EVALUATION VISIT – Describes information needed to determine the appropriateness of an arm support for the patient and provides an example Letter of Medical Necessity.
- INSTALLATION VISIT – Form to document user and caregiver training and to assess user function without and with the arm support. Establishes patient objectives and assesses current condition.
- FOLLOW-UP VISITS – Form documents exercises performed and progress in functional improvement and objective improvement in reaching patient goals.
